Acute Scrotum

In this section, we discuss different causes that can generate the same symptom, which is intense, abrupt testicular pain, among other specific causes depending on the etiology of the pain. Causes can range from testicular torsion, hydatid torsion of Morgagni, or epididymo-orchitis.

Testicular torsion can occur at any age, although it is most common between the ages of 12 and 18. It is considered a medical emergency in which the patient requires prompt and priority care. The torsion interrupts blood flow to the testicles, causing swelling, nausea and vomiting, abdominal pain, sweating, and elevation of the affected testicle. It is necessary to consult a urologist within the first 4 hours to limit the damage and preserve the testicle.

Torsion of the hydatid of Morgagni: This structure is an embryonic remnant containing a large vascular supply and fibrous tissue. Torsion of this structure can cause pain that is not as intense as that of testicular torsion and is not associated with general symptoms. It is confirmed by physical examination and ultrasound. If the condition is mild, it is treated only with anti-inflammatory drugs and occasionally surgical treatment for hydatidectomy. This differentiation will be made by a urology specialist.

Epididymitis-orchitis is the inflammation of the epididymis, testicle, or both as a result of a bacterial or viral infection, trauma, sexually transmitted diseases, or other conditions. This condition manifests as pain and tenderness in the scrotum, redness, fever, irritating symptoms during urination, mild enlargement of the testicle, and induration. Diagnosis is clinical and confirmed with ultrasound and urine tests. Treatment is directed toward the underlying cause, which may include antibiotics and anti-inflammatory drugs, and surgery in the last case, where the infection causes abscesses.
